Hospital Handover

The Hospital Handover Delays Collaborative programme at NWAS used an approach first described by the Institute for Healthcare Improvement (IHI) called the Breakthrough Series.

NWAS developed 15 steps to delivering a successful collaborative, shown below. The shared learning is that for a breakthrough collaborative to work all 15 have to be continuously addressed, this an intensive programme of work.

NWAS have share the learning and outcomes across the NAA as shown below.
